How do I nominate a nurse for the Daisy Award?
Each DAISY Award Honoree will be recognized at a public ceremony in her/his unit each quarter, and will receive: Veterans, families, visitors, nurses, physicians, and other employees may nominate a nurse by completing and submitting the nomination form .
If there is more than one nurse you would like to nominate, please list them on the form with their names separated by commas. DAISY also has a Team Award that honors nurse-led teams.
